Van Driver Safety Training Registration

This form is to register for Van Driver Safety training, which must be completed prior to driving a campus van. Please fill out the information below.

  • LENS Check Verification

    In order to drive a vehicle owned by SUNY Brockport, you must be "LENS Valid". To have a LENS check performed on your DMV record, you must fill out the form at https://forms.brockport.edu/view.php?id=4810355.

  • Defensive Driving course

    Drivers under 25 must take DMV-approved defensive driving course and file proof of this with the office of Environmental Health and Safety. Eligible courses may be found at https://dmv.ny.gov/tickets/point-insurance-reduction-program.
