This form is for current Brockport students who would like existing transfer credit from their degree audit (Degree Works) reevaluated. Current students may also use this form to see how courses not already transferred to the University may be evaluated. An official syllabus for the transfer course must be uploaded to this request to receive an accurate evaluation. Faculty and staff wishing to update a credit equivalency may complete the Faculty/Staff Transfer Credit Update form. *Please Note: For Undergraduate Students ONLY - Graduate credit requests will not be processed. If you have credits you would like evaluated for your graduate degree, please fill out the Graduate Student Transfer Credit Request form.
