This form is required when seeking signature and stamp on transcript/diploma or both.
An apostille request is when a student is requesting a certified transcript or diploma as proof to an international institution or organization confirming SUNY Brockport has the legal authority to grant degrees.
Steps to follow -
1- go to www.parchment.com - order your transcript or diploma or both.
Have your transcript/diploma sent by Parchment to

2. Call 585-395-2531 indicating you need an Apostille alerting us that you have initiated this request.
Please note Parchment takes 3-6 weeks for a diploma to be printed and sent. Upon receipt SUNY Brockport will do their processing piece of the request and then mail the document(s) back to you.
3. Once you have received document(s) back from SUNY Brockport you should - Contact Monroe County and complete the APOSTILLE REQUIREMENTS FOR CERTIFICATES TO BE USED IN FOREIGN COUNTRIES
Per the Monroe County website:

STEP 2
Send Certificate to Monroe County Clerk’s Office the obtain an “Oath of Office Certificate” Monroe County Clerk Attn: Oath of Office 39 W. Main Street Rochester, NY 14614 585-753-1600 Fee: $5.00 (check/money order)
STEP 3
