Once the key request has been completed,
● You will be notified through email and you may pick them up at the Facilities Service Center between 8 AM - 4 PM.
● Only the person being issued the key(s) can pick up the key(s). Please be prepared to show your ID.
● Keys will only be kept in the Service Center for four (4) months; after that date they will be returned to the Lock Shop and a new key request will be required.

1. The GA/TA supervisor must complete this form as the person "submitting this form for someone else".
2. Enter GA/TA information in all "Receiver of the key" fields.
3. Indicate that the receiver of the key is "Staff".
Faculty/Staff or affiliates who submit a request for GA/TA keys are responsible for the return of those keys.
