If you are not going to be on campus during a specific week, use this form to apply for a one-time exemption. You must submit this form during the week that you are not going to be on campus and no later than 5pm on Friday of that same week. By submitting this request, you are verifying that you will not be coming to campus for any reason during the upcoming week. You should not submit this request if you still intend to come to campus for other reasons because you will still be required to participate in weekly testing (library, fitness center, etc).
