Permission to Enroll (Late PTE) AFTER the LATE ADD

  • Additional permission is needed to add a class after the late add period has ended. THIS FORM IS INITIATED BY THE STUDENT

    ● You will need permission from: (a) the INSTRUCTOR, and (b) the appropriate DEAN'S OFFICE.

    ● CONFIRM THE ADD PERIOD HAS ENDED FOR THE COURSE YOU WISH TO ADD WHEN USING THIS FORM.

    ● TO DETERMINE IF IT IS AFTER THE LATE ADD PERIOD, REFER TO THE DATES AND DEADLINES on the “Dates and Deadlines” web page.

    ● A $20 LATE ADD FEE WILL BE ASSESSED.

    ● TO BE SURE YOUR COURSE INFORMATION IS ACCURATE, PLEASE REFER TO THE Course Schedule.
