How to receive credit by examination:
1. Review the "Departmental Credit by Examination" section of the Getting the Credit You Deserve policy.
2. Student requesting credit completes and submits this form.
3. Upon submission, this form is automatically routed to the department chair for approval.
4. Student is notified by email of approval; attached to the email is a "Credit by Examination Fee Assessment" document (PDF).
5. Student prints the PDF.
6. Mail (or deliver) PDF with payment to the Office of Student Accounts and Accounting (OSAA).
7. After payment is processed, OSAA will notify the academic department to schedule the exam.

STUDENT ACKNOWLEDGEMENT/AGREEMENT
I understand that:
● Credit by examination requires a $295 fee per course payable to SUNY Brockport before the examination is taken.
● Credit by examination is available to matriculated undergraduate students only.
● I must obtain a grade of “C” or better in the examination to be granted credit.
● If credit is granted, the number of credit hours awarded and the notation “credit by exam” will be recorded on the transcript. The degree audit will show the course completed through credit by examination. If the examination is failed, no entry will be made on the transcript or degree audit.
● I may take an examination for credit only once in this course.
