Request to change a course(s) from a letter grade to satisfactory/unsatisfactory option.

    I understand that changing to satisfactory/unsatisfactory grading option will require a C* or better to earn a satisfactory "S" grade and may impact the following:

    * Graduation
    * Financial aid
    * Scholarship
    * Graduate School admission
    * Transferability of the course(s)
    * Accreditation
    * Overall GPA

    I have discussed these issues with my advisor and or Financial Aid staff and request this change. Once chosen at the end of the semester, students may not request the grades be changed back to the original letter grades.

    The deadline to submit this form is Midnight on January 28, 2021.

    *At the graduate level, an S grade requires a B or better.
