All SUNY Brockport departments and offices are required to maintain a
Continuity of Operations Plan (COOP)
to ensure that essential services and academic operations can continue—or resume quickly—during and after a disruption.
This survey guides departments in identifying essential functions, leadership succession, communication procedures, and recovery strategies to ensure the University can continue critical operations during and after a disruption. Each section helps your department document how it will
sustain essential activities, protect personnel, and recover efficiently.
Please read the instructions below carefully before completing each section.
Who Should Complete This Survey
The department head or a designated COOP contact (such as an assistant director or administrative manager) should coordinate completion of this document. Input should be gathered from supervisors, key staff, and anyone responsible for essential operations, information systems, or safety compliance.
How to Complete the COOP Survey
1. Complete all sections (B–H)
. Provide concise but detailed responses that accurately reflect current operations.
2. Collaborate internally
—consult with your leadership team, administrative staff, and any functional area that supports essential functions.
3. Be specific and actionable
—general answers (e.g., “IT will handle this”) do not meet continuity standards. Identify roles, systems, and dependencies.
4. Use plain language
that is easy to understand by anyone who may need to activate the plan.
5. Update your COOP annually
or when there are changes in leadership, staffing, or operations.
6. Submit electronically
and retain a copy in both digital and printed formats for departmental reference.

A. Background
Emergencies can occur with little or no warning, disrupting teaching, research, and administrative operations.
Examples include:
• Severe weather and utility outages
• Fire or hazardous materials incidents
• Cyber or technology disruptions
• Public health emergencies
• Security threats or acts of violence
A well-developed COOP ensures that SUNY Brockport can:
1. Protect life and safety
of students, faculty, and staff.
2. Secure critical infrastructure and facilities.
3. Maintain and resume essential functions
with minimal disruption.
Departments play a key role in supporting these institutional goals. This survey helps your unit identify:

Essential Functions:
Activities that must continue or quickly resume to sustain operations.
•
Mission Essential Functions (MEFs):
The most critical activities that cannot stop or must restart immediately to prevent severe impact on health, safety, academics, or institutional continuity.
•
Dependencies, staffing requirements, and recovery resources
that support those functions.
Your completed COOP serves as both a
planning document and operational reference
during incidents that affect normal operations.
