This Online Faculty Approval Form allows the Office of Special Sessions & Programs and the Academic Support team to:
• determine your eligibility for a course development stipend.
• help you sign up for professional development and training you will need to teach online.
The process:
1. Consult with your chair/director your intent to teach a 100% online course.
2. Complete all parts of this form. You can save your work and return to it later.
3. Once submitted, your form will be forwarded to your Department Chair/Director and Dean for approval. You will receive a copy of the submitted form.
4. You will be contacted by email regarding your training request (if applicable) after your Chair/Director and Dean approve the Online Faculty Approval Form.
